Available Positions 

Community Residence Aides: Full-time and Relief Work

Shifts Saturday-Sunday: 7am - 3pm; 3 -11pm; 11pm - 7am.  

Community Residence Aides provide supervision and support services to individuals with a mental health diagnosis during day, evening & weekend shifts in a mental health residential setting. Ideal candidates will have experience working with adults with serious mental illness. Assist consumers in group home with ADL’s and skill building, recreation & leisure activities, monitor medication compliance and document to standards. Transport clients to programs, activities and appointments, as needed. Assist in linking clients with needed community resources, particularly in the areas of health and dental care, mental health care, education, job training, and socialization, as directed; promptly report any problems with service linkages to the Senior Residence Counselor.  Ensure sites are kept cleaned and maintained.
QUALIFICATIONS: A Bachelor's degree in a Human Services field (OR, junior/senior standing), OR, Associate’s degree plus one (1) year of related experience, OR, High School diploma plus three (3) years health/human services experience, OR, CASAC. Valid NYS Driver’s License.
Rate: $10.50/hr.

Home and Community Based Services Instructor (Per Diem, Various Hours)

The Home and Community Based Instructor provides instruction and supervision to individuals participating in the HCBS Program. The Community Based Instructor works with individuals in their own homes and in the community to increase their independence in activities of daily living. QUALIFICATIONS: A Bachelor degree in a Human Services field (OR, junior/senior standing), OR, Associate’s degree plus one (1) year of related experience, OR, High School diploma plus three (3) years health/human services experience, OR, CASAC. Valid Driver’s License. Must have the ability to transport individuals in personal vehicle that meets agency requirements. 
Rate $10.50/hr

 

Community Housing Program (CHP) Case Manager 

Provides person-centered services for participants in the community housing program.  Services include:  conducting enrollment and assessment, identifying available apartments and resources based on individual assessment and establishing a service plan.  Requirements: Master’s/Bachelor’s degree in Social Work, Human Services or related field plus two (2) years paid experience working with individuals with handicapping disabilities; Must have a flexible schedule; Must undergo and pass a criminal background check.  FT. Candidate must have a car and a valid NYS driver’s license. Salary: Low 30’s
 

Community Residence Aide (CRA) Team Leader 

Provides assistance to the Senior Residence Counselor by assisting with preparing for Office of Mental Health site visits, by ensuring that sites and charts are in audit ready condition at all times; Review daily notes on a routine and regular basis; Assists the Senior Residence Counselor in the develop and implementation of  Individual Service Plans in cooperation with direct care staff; assist staff in working with residents toward identified goals; Regularly and routinely document resident progress  toward goals in accordance with Agency policy.  Provides direct rehabilitation services to residents. Develops and maintains effective working relationships between and among residence staff and other service providers. Requirements: Qualified Mental Health Professional (QMHP) with Bachelor’s degree in Social Work, Human Services or related field plus one (1) year paid experience working with individuals with handicapping  disabilities; or, Associate's degree in Human Services plus three (3) years’ experience as above; or, High School diploma plus five (5) years’ experience as above; able to demonstrate at least (1) one year of paid leadership experience; must have a flexible schedule; must undergo and pass a criminal background check.  FT. must have reliable transportation and a valid NYS driver’s license.
Rate: $13/hr.

 

Intake Specialist (Part-Time) 

Responsible for completing telephone admissions process; organizing and maintaining the collection and storage of clinical information related to individuals requesting services. Obtains documentation of disability and submits to the appropriate regulatory agencies for service eligibility approval. Assists in maintaining Admissions databases. Master’s/Bachelor’s degree in Social Work, Human Services or related field plus two (2) years paid experience working with individuals with handicapping disabilities; Must have a flexible schedule; Must undergo and pass a criminal background check.  Candidate must have a car and a valid NYS driver’s license.
 

Senior Residence Counselor (SRC)

Facility operation and maintenance--Assure that standards of housekeeping, maintenance, health, safety and general appearance of the residence are met; inspect facilities on a regular basis and report necessary repairs and equipment replacement in a timely manner. Prepares for Office of Mental Health site visits, by ensuring that sites and charts are in audit ready condition at all times. Develop and implement Individual Service Plans in cooperation with direct care staff; assist staff in working with residents toward identified goals; regularly and routinely document resident progress toward goals in accordance with Agency policy.  Requirements: Qualified Mental Health Professional (QMHP) with Bachelor’s degree in Social Work, Human Services or related field plus one (1) year paid experience working with individuals with handicapping disabilities; or, Associate's degree in Human Services plus three (3) years’ experience as above. Must be computer literate (Word, Excel, Outlook); Experience in developing service plans and conducting assessments. Must have a flexible schedule, NYS Driver's license; must have reliable transportation.
