Case Management Services

BFNC's Case Management Services has four mental health case management programs to meet the needs of consumers.  These programs provide a continuum of care with maximum flexibility and the option to move easily and efficiently from one level to the next.




Benefits of Case Management:

  • 95 percent of contacts take place in the consumer's environment.
  • Our "Person Centered Approach" helps consumers identify and implement their own visions of recovery and includes their desires and interests.
  • We work in harmony with all Erie County providers, adapting to various philosophies and treatment approaches.

Consumers are expected to:

  • Show willingness and commitment to work on the agreed upon recovery goals.
  • Work with us to identify and change behaviors that may be keeping them from realizing their full potential.
  • Consider, learn and apply new life skills.

Services Include:

  • Assistance applying for financial entitlements.
  • Referrals for housing, vocational or educational needs.
  • Linkage with mental health, substance abuse, or physical health services.
  • Referrals for clothing, transportation, household goods and food.



Community Support Services (CSS):

For recovering consumers who need periodic assistance and low-level monitoring and support to help maintain their current level of functioning and rehabilitative gains.  Services provided Monday through Friday, 9 a.m. to 5 p.m.; contact with the case manager minimum once a month; ongoing follow-up with other service providers.

Supportive Case Management (SCM):

For individuals who tend to have a higher risk and needs beyond Community Support Services (CSS).  Face-to-face contacts minimum of twice monthly.  Ongoing coordination and case conferences with other service providers.

Transitional Case Management (TCM):

For individuals transitioning from a correctional setting.  The needs of these consumers are often acute, urgent, and manageable within approximately nine months.  Face-to-face contacts are provided minimum twice monthly with ongoing coordination and case conferences with other service providers, particularly criminal justice system providers.

Intensive Case Management (ICM)

For individuals with the most serious functional disabilities due to mental illness.  These individuals tend to have the highest risk and have the highest need for intense services, small staff-to-consumer ratio; personal contact minimum four times per month or more as needed; ongoing follow-up and coordination with other service providers.

 

Working with BFNC's Case Management:

Ask for a referral from your hospital or mental health care provider to receive Case Management services from BFNC.
